Who decided three meals a day was best?

The historian Abigail Carroll, author of "Three Squares: The Invention of the American Meal," says the practice goes back to the Middle Ages, but got a boost from the bigotry of the first colonists as well as later when cereal companies pushed the idea of eating their products to start the day.
